To make a online report, select the correct option below.

Sexual Misconduct or Title IX Violation

Including sexual assault, dating/domestic violence, and stalking

Harassment, Discrimination, or Retaliation

On the basis of any protected class 

Incident of Bias 

Does not rise to the level of harassment, discrimination, or retaliation

Reporting a Crime 

If you have witnessed a crime on campus or have information concerning a crime or potential crime

Other Incident, Injury, or Safety Concerns 

Non-employee injury, employee injury, or other unsafe conditions on campus

Ethics Point Reporting 

Inappropriate activity relating to financial matters, research, risk or safety, data and information, or other related matters

          Reporting a Free Speech Concern

                   Violation of freedom of expression (speech, writing, publishing, etc.)

All administrators, faculty, and staff have the obligation to report incidents of sexual misconduct involving students to the UNI Title IX Officer or the UNI Title IX Deputy Coordinator. For more information about reporting responsibilities, click here.
