Sexual Misconduct or Title IX Violation
Including sexual assault, dating/domestic violence, and stalking
Harassment, Discrimination, or Retaliation
On the basis of any protected class
Does not rise to the level of harassment, discrimination, or retaliation
If you have witnessed a crime on campus or have information concerning a crime or potential crime
Other Incident, Injury, or Safety Concerns
Non-employee injury, employee injury, or other unsafe conditions on campus
Inappropriate activity relating to financial matters, research, risk or safety, data and information, or other related matters
Reporting a Free Speech Concern
Violation of freedom of expression (speech, writing, publishing, etc.)
